Patient Profile<\/b><\/p>\n

    \n
  • woman born in 1974<\/li>\n
  • First Meeting: March 2016<\/li>\n
  • Referral through a very good friend, who sought a seemingly \"random\" meeting at a social event<\/li>\n
  • History of heavy generalized endometriosis<\/li>\n
  • Extensive intra-abdominal adhesions<\/li>\n
  • Multiple previous operations<\/li>\n
  • serious burden on quality of life for many years<\/li>\n<\/ul>\n

    history of disease<\/b><\/p>\n

    The patient had a history of generalized endometriosis, with extensive abdominal involvement and multiple adhesions.<\/p>\n

    The disease had significantly affected the abdominal organs, causing severe symptoms and chronic daily burden.<\/p>\n

    Since 2008, due to endometriosis, during each menstruation it bleeds internally in the abdomen. The result was severe pain, which lasted about 10 to 15 days in each cycle of 28 to 30 days.<\/p>\n

    In practice, almost half of her life every month was determined by pain.<\/p>\n

    symptoms<\/b><\/p>\n

      \n
    • Very severe abdominal pain during menstruation<\/li>\n
    • Internal bleeding in the abdomen due to intrauterine foci<\/li>\n
    • Symptoms of 10 to 15 days per cycle<\/li>\n
    • Need to use strong painkillers<\/li>\n
    • Inability to get out of the house during painful days<\/li>\n
    • inability to work<\/li>\n
    • Staying in bed for several days each month<\/li>\n
    • Absence of sexual intercourse with the spouse for 8 years due to severe symptoms<\/li>\n<\/ul>\n

      The situation had affected not only her physical health, but also her work, everyday life, psychology and marital relationship.<\/p>\n

      clinical challenge<\/b><\/p>\n

      The incident was extremely difficult, as the disease was generalized and the abdominal anatomy had been altered by extensive adhesions.<\/p>\n

      The abdominal organs were stuck together, which made any surgical intervention of extremely high risk.<\/p>\n

      Although an operation was not theoretically impossible, the possibility of a serious complication was very high.<\/p>\n

      Colleagues with extensive experience in severe cases of endometriosis were negative to proceed with surgical treatment, precisely because of the extent of the disease and the risk of severe complications.<\/p>\n

      This is an incident where persistence in a surgical solution could create a bigger problem than it was trying to solve.<\/p>\n

      The meeting<\/b><\/p>\n

      The first meeting took place in March 2016, at a social event, following the initiative of a very good friend of the patient.<\/p>\n

      Within the first few minutes of the conversation, it became clear that the patient lived for years in a way that should not be considered acceptable.<\/p>\n

      Pain had become part of her life. The inability to work had become routine. Isolation during the days of the period had become a given. The absence of sex life with her husband had lasted eight years.<\/p>\n

      And yet, the solution was not necessarily a new, heavy and dangerous operation.<\/p>\n

      Therapeutic Thinking<\/b><\/p>\n

      The root cause of the symptoms was the cyclic activation of the intrauterine foci during menstruation.<\/p>\n

      Simply put, every month, the endometriosis foci acted like an endometrium outside the uterus, causing bleeding inside the abdomen and a strong inflammatory reaction.<\/p>\n

      Therefore, the therapeutic objective was clear:<\/p>\n

        \n
      • to stop menstruation<\/li>\n
      • Stop cyclic bleeding of endometriotic foci<\/li>\n
      • to reduce pain<\/li>\n
      • to avoid an extremely dangerous surgery<\/li>\n
      • maintain good hormonal support without period challenge<\/li>\n<\/ul>\n

        Diagnostic & Therapeutic Approach<\/b><\/p>\n

        It was explained to the patient that a simple pharmaceutical manipulation could be applied, with the aim of inhibiting menstruation.<\/p>\n

        At the same time, it could receive non-estrogenic hormone replacement, which does not cause periods.<\/p>\n

        This particular approach was aimed at stopping the mechanism that caused the severe symptoms, without the patient undergoing a new high-risk surgery.<\/p>\n

        The treatment was designed based on:<\/p>\n

          \n
        • the gravity of endometriosis<\/li>\n
        • the inability to safely treat<\/li>\n
        • the need for immediate pain relief<\/li>\n
        • restoring daily functionality<\/li>\n
        • improving the quality of life<\/li>\n
        • protect the patient from unnecessary surgical risk<\/li>\n<\/ul>\n

          Result<\/b><\/p>\n

          The response was impressive.<\/p>\n

          From the first month after the start of treatment, the patient experienced complete remission of symptoms.<\/p>\n

          The pain that pinned her down for 10 to 15 days each month stopped.<\/p>\n

          Her daily life was restored.<\/p>\n

          The patient returned to work normally and was able to have a normal life again.<\/p>\n

          Within the first trimester she again began to have comfortable sexual intercourse with her husband, while the couple's sex life was completely restored within the first semester.<\/p>\n

          clinical conclusions<\/b><\/p>\n

            \n
          • Heavy endometriosis does not always require a surgical solution<\/li>\n
          • In highly symbiotic cases, surgical intervention can have a very high risk of complications<\/li>\n
          • A correct understanding of the mechanism of pain can lead to simpler and safer solutions<\/li>\n
          • The inhibition of menstruation may be decisive in selected cases of endometriosis<\/li>\n
          • Treatment should target not only the disease, but also the quality of life of the patient<\/li>\n
          • Success is not measured only by surgical findings or laboratory values, but by the return of the woman to her life<\/li>\n
          • In some cases, avoiding a dangerous operation is the most correct treatment option<\/li>\n<\/ul>\n

            What is endometriosis<\/b><\/p>\n

            Endometriosis is a serious gynecological disease, in which tissue similar to the endometrium grows outside the uterine cavity.<\/p>\n

            The endometrium is the tissue that grows naturally inside the uterus during the cycle and falls off with the period.<\/p>\n

            In endometriosis, similar tissue can be found in places such as:<\/p>\n

              \n
            • the ovaries<\/li>\n
            • the gut<\/li>\n
            • the abdominal wall<\/li>\n
            • The fallopian tubes<\/li>\n
            • The peritoneum<\/li>\n
            • or other parts of the abdomen<\/li>\n<\/ul>\n

              These foci can bleed cyclically, like the endometrium during the period, causing inflammation, adhesions and severe pain.<\/p>\n

              Over the years, adhesions and inflammation may worsen, leading to an even greater burden on the patient.<\/p>\n

              Final message<\/b><\/p>\n

              There are cases where the right treatment is not the most impressive or the most invasive.<\/p>\n

              She is the one who understands exactly the mechanism of the problem and intervenes at the right point.<\/p>\n

              For this patient, the solution was not yet another heavy surgery.<\/p>\n

              It was the interruption of the cycle that each month reproduced pain, bleeding, immobilization and isolation.<\/p>\n

              The complete remission of symptoms from the first month and her return to work, relationship and normal life show that sometimes the most essential medical procedure is to avoid the
